The State Workforce Authority (SWA) agency will ask for the following information for your business.
And they will want to receive it from your business email address.
We will request most of this information from you through your Attorney after you complete the SWA Questionnaire Form they sent you.
Here’s is what we have observed over the last 18 years from SWA agency staff in response to Account Registrations:
1. SWA Agency staff send the following request:

Please email a document copy of your “Articles of Incorporation in the State of Indiana” that was sent to you when the company registered with the State of Indiana.
Please confirm the following by answering the questions below, then reply by email your answers.

- Company email:
- Company Website:
- Number of Employees (Company Size):
- Are you a Recruiting Employer?
- A second contact (Title, Phone Number and Company email):
2. We / you receive an email from SWA representative saying they tried calling but couldn’t get in contact so please contact him / her back.
3. We / you receive No email response whatsoever from the SWA Agency staff.
If after 48 hours have passed since Indiana Career Connect account registration, we take the following action:
We utilize existing SWA Agency staff contact information in our database, and send the Attorney and Petitioner a pre-written Help SWA Agency Request email / phone script that can be sent to the SWA Agency staff.

According to our information, two business days since Registration on Indiana Career Connect Online was completed, but our client has not heard back from your Agency either by Phone or Email.
We politely wish to expedite resolution of any potential Indiana Career Connect Online Account Verification you may have, if any.
May we request your assistance or referral to the proper staff to mitigate this issue, so that our clients Indiana Career Connect Online account can be approved and enabled, such that they may post the above-referenced mandatory 30 Day Job Order?
