The State Workforce Authority (SWA) agency will ask for the following information for your business.
And they will want to receive it from your business email address.
We will request most of this information from you through your Attorney after you complete the SWA Questionnaire Form they sent you.
Here’s is what we have observed over the last 18 years from SWA agency staff in response to Account Registrations:
1. SWA Agency staff send the following request:
PLEASE ANSWER AND FOLLOW ALL THE REQUIREMENTS BELOW!
Please email a document copy of your “Articles of Incorporation in the State of New Hampshire” that was sent to you when the company registered with the State of New Hampshire.
Please confirm the following by answering the questions below, then reply by email your answers.

- Your position title:
- Telephone number:
- E-mail address:
- Company Physical Address:
- Company email:

2. We / you receive an email from SWA representative saying they tried calling but couldn’t get in contact so please contact him / her back.
3. We / you receive No email response whatsoever from the SWA Agency staff.
If after 48 hours have passed since NHWorks account registration, we take the following action:
We utilize existing SWA Agency staff contact information in our database, and send the Attorney and Petitioner a pre-written Help SWA Agency Request email / phone script that can be sent to the SWA Agency staff.
An example of such an email is below:
