The State Workforce Authority (SWA) agency will ask for the following information for your business.
And they will want to receive it from your business email address.
We will request most of this information from you through your Attorney after you complete the SWA Questionnaire Form they sent you.
Here’s is what we have observed over the last 18 years from SWA agency staff in response to Account Registrations:
1. SWA Agency staff send the following request:
We have reviewed your request to register with Utah Department of Workforce Services but must place your request on temporary hold for the following reasons:

- Pending verification of your account details
- Email a copy of the SS4 letter from the IRS from when the EIN account number was assigned.

2. We / you receive an email from SWA representative saying they tried calling but couldn’t get in contact so please contact him / her back.
3. We / you receive No email response whatsoever from the SWA Agency staff.
If after 48 hours have passed since Utah Department of Workforce Services account registration, we take the following action:
We utilize existing SWA Agency staff contact information in our database, and send the Attorney and Petitioner a pre-written Help SWA Agency Request email / phone script that can be sent to the SWA Agency staff.
