The State Workforce Authority (SWA) agency will ask for the following information for your business.
And they will want to receive it from your business email address.
We will request most of this information from you through your Attorney after you complete the SWA Questionnaire Form they sent you.
Here’s is what we have observed over the last 18 years from SWA agency staff in response to Account Registrations:
1. SWA Agency staff send the following request:
In order to complete the registration and to post your job listing, please email me the following information and reference it to Employer ID# please:

- Your company’s State of Oregon Tax ID number, the one you use to pay into unemployment insurance taxes. Your payroll person should be able to provide it; it can also be found on line 15 of your W2 form.

2. We / you receive an email from SWA representative saying they tried calling but couldn’t get in contact so please contact him / her back.
3. We / you receive No email response whatsoever from the SWA Agency staff.
If after 48 hours have passed since WorkSource Oregon account registration, we take the following action:
We utilize existing SWA Agency staff contact information in our database, and send the Attorney and Petitioner a pre-written Help SWA Agency Request email / phone script that can be sent to the SWA Agency staff.
