The State Workforce Authority (SWA) agency will ask for the following information for your business.
And they will want to receive it from your business email address.
We will request most of this information from you through your Attorney after you complete the SWA Questionnaire Form they sent you.
Here’s is what we have observed over the last 18 years from SWA agency staff in response to Account Registrations:
1. SWA Agency staff send the following request:
This is an effort to reduce fraudulent activity to WorkSourceWA employer account requests. To complete and approve your request for an employer account, we need to verify you have the authority to make this request on behalf your company. Verification must be provided by emailing us a copy of the Federal EIN registration document and providing your Washington State unemployment insurance tax ID number.
This account request will be denied in 10 days if we do not receive the requested information.
WE CANNOT ACCEPT UBI or W9 TAX FORMS AS VERIFICATION OF THE FEIN NUMBER.
2. We / you receive an email from SWA representative saying they tried calling but couldn’t get in contact so please contact him / her back.
3. We / you receive No email response whatsoever from the SWA Agency staff.
If after 48 hours have passed since WorkSource WA account registration, we take the following action:
We utilize existing SWA Agency staff contact information in our database, and send the Attorney and Petitioner a pre-written Help SWA Agency Request email / phone script that can be sent to the SWA Agency staff.

Our law firm is representing Petitioner Point of Contact Name (Owner) and their company, Example Business LLC, on a PERM Labor Certification matter.
One of the requirements of PERM Labor Certification per CFR 656.17 is to Post a 30 Day SWA Job Order on your WorkSource WA Online site.
On the date of July 15, 2022, our client Registered their company on the WorkSource WA Online site, using the following User ID and business credentials:
- Company Name: Example Business LLC
- Company FEIN: 12-3456789
- Company State UI#: 123456
- Address: Example Street, 2nd Floor, Example City, State, Zip Code
- Contact Person: Example Person (Owner)
- Phone: 213-456-7890
- Email: firstname.lastname@example.org
According to our information, two business days since Registration on WorkSource WA Online was completed, but our client has not heard back from your Agency either by Phone or Email.
We politely wish to expedite resolution of any potential WorkSource WA Online Account Verification you may have, if any.
May we request your assistance or referral to the proper staff to mitigate this issue, so that our clients WorkSource WA Online account can be approved and enabled, such that they may post the above-referenced mandatory 30 Day Job Order?
